The Challenge
CHOICE supports lives across 7 counties in Western Washington through a network of community-based workers and over 20 partner organizations. But the most valuable work these teams did every day was also the least visible. Care workers spent the majority of their time on documentation, referrals, and compliance instead of direct care. Disconnected systems meant fragmented data and delayed reporting. And the deepest gap wasn't efficiency — it was visibility. Handwritten notes and spreadsheets captured roughly 70% of the work being done. The rest: the connections made, the needs surfaced, the people helped in ways no form recorded, simply disappeared. For the populations facing the steepest disparities, invisible work means invisible needs — and invisible needs don't get funded.
The Solution
Elora was deployed across hospital discharge, telehealth, mobile health, and human services teams. Ambient AI documentation — captured with the client's consent during the visit, or by voice memo after — eliminated the documentation backlog without pulling workers out of the relationship. Real-time resource navigation replaced manual referral research. Automated, audit-ready reporting gave program directors clean data without manual gathering. The result: more time with clients, coordinated records across agencies so people aren't asked to repeat their story at every door, and for the first time, a near-complete picture of the work the network actually does.
"There's an opportunity in this to drive payers and health systems to greater visibility, do more with less, and improve outcomes along the way.
— JP Anderson, CEO, CHOICE Regional Health Network
